As another uk person, I think fine is fine. We have some chatty cashiers and some grumpy teenagers who just glare at you. Mostly, at my local supermarket, they will say "Hi, how are you?" I will say "I'm ok, it's a bit busy" etc and try to politely strike up some conversation about the weather or the amout of people in the shop as I may be bagging my items for a while and I think it is not polite to ignore this poor person. Obviously, if it is a sullen teenager, I will say "fine" and let them be grumpy. My son, same age, is expected to say "fine thanks" as a response. He has hit an age where he seems quite shy and he feels embarassed by being asked this, especially by a women. I would not make him say how are you, as it is likely to make him feel more uncomfortable and this person probably doesn't care in the slightest how your son is and can't be bothered to think of a response. I think it is different from being asked this by a family member where I would expect him to ask how they were etc.
